Main Purpose

Organize and carry-out all nursing activities related to the operating theatre (OT) running, according to MSF health protocols, universal hygienic standards and in coordination with the surgeon and anaesthetist, in order to ensure a smooth functioning of the surgical activities (pre, during and post-operative care) offered to patients.

Accountabilities

•          Ensure the preparation of the material and equipment (checking the quantity and quality), in the OT and carry-out the reception, transport of the patient from the preparation room to the OT and ensuring his/her installation for surgery.

•          Assist the surgeon and anaesthetist during the operation, anticipating their needs, in order to facilitate their job and helping throughout the whole process.

•          Checks vital parameters of the patient with the nurse anaesthetist

•          Organize the patient’s transport and installation from the operating theatre to the recovery room and carry-out and/or supervise all the post-operative activities (used material collection, OT’s cleaning and disinfection, resetting all equipment and material back), in order to prepare the OT for next surgical intervention or any emergency.

•          Implement and follow-up the hygiene, sterilization, asepsis and disinfection protocols and procedures at all times, in pre, during and post-operative activities, in order to ensure the patient and staff’s safety conditions. i.e. waste containers availability, sharps container, checking no material or needles remain in the surgical towels, placing instruments into soak container, take instruments/linen to sterilization unit, etc.).

•          Supervise and train the nurse-aids and cleaners while in the OT in following all appropriate protocols and procedures, in order to ensure the quality of the healthcare given to patients. and train NS where applicable

•          Perform cleaning and minor maintenance for biomedical equipment used. Follow user manual and protocols and alert supervisor in case of malfunctioning of any device.

•          Control all equipment and material stock to be used in the OT, in order to ensure an efficient and rationale use of material resources while preventing damage and deterioration.

•          Carry-out and supervise administrative procedures and documents (check identity of patient and his/her consent for the operation, fill in patients files, forms, statistics, quantitative surgical data collection, etc.), and report any problematic situations and cases arising, in order to have updated and correct information about the day-to-day activity and ensuring proper transfer of information.

•          Participate in data and reporting

•          Intervene in support of other services whenever needed and work particularly in close collaboration with the accident, emergency department and with obstetrics.

MSF Section/Context Specific Accountabilities

Pre-operative tasks:

Preparation of the Operating room:

·       Cleans the OT room, before the surgical program begins, between each patient and when the surgical program has ended, following MSF protocols.

·       Perform the hygiene check by using the hygiene checklist. 

·       Provide and prepare all instruments and equipment needed for the surgical interventions of the day.

·       Ensure all instruments and equipment are ready-to-use and available in sufficient quantity in accordance with the needs of the next surgical interventions.

-  Verify biomedical equipment functions properly and safely.

-  Verify the sterile condition of sterile materials (date, packaging integrity, colours of TST strip).

·       Write down before the start of the intervention the total number of compresses, the type and number of surgical sets and of every other specific instrument that was provided.

Transport and reception of the patient:

·       Meet the patient at the entrance of the OT.

·       Ensure the patient’s medical file is complete together with the nurse from the ward.

·       Verify the identity of the patient.

·       Ensure authorization for the intervention was given and an authorization file was signed- Consent.

·       Check the patient has no known allergies (iodine, penicillin, ...) and inform the surgical team about it.

·       Explain the procedure to the patient and answer their questions.

·       Check pre-operative hygiene measures were taken (pre-operative shower, removal of jewelry, clean bed linen and clothes,...).

·       Organize safe, comfortable, and on-time transport from the ward to the OT.

·       Is responsible for the quality of nursing care in the preparation room. 

·       Make sure nursing care needed for good preparation of the patient and his comfort are provided.

·       Ensure nursing care meets MSF recommendations and standards. 

·       Organize safe transport for the patient from the preparation room to the operation room.

·       Is responsible for adequate and safe installation of the patient on the operation table.

·       Connect the patient to the monitoring equipment.

·       Monitor the patient till an anesthetist or surgeon is ready to take over.

·       Is responsible for the nursing care in the operation room.

·       Ensure nursing care meets MSF recommendations and standards.

·       Respect patients' privacy, comfort, and dignity at all times.

Provide assistance

·       Assist  the surgeon and the scrub nurse.

Intra-operative tasks:

·       Ensure the good progress of the surgical intervention

·       Anticipate the needs of the surgical team during the intervention.

·       Prepare, control and handle non- sterile medical devices.

·       Present in an aseptic way sterile devices.

·       Supervise the respect of standard precautions by all staff members.

·       Watch that all staff members respect the rules of asepsis during the surgical intervention.

·       Minimize and keep to a minimum the number of people allowed to enter the OT during the procedure. 

·       Note the exact number of compresses and materials that were supplied additionally during the intervention.

·       Count all the compresses, needles, sutures, and surgical instruments before the operating site is closed and inform the surgeon of the amount. (scrub nurse and circulating nurse together)

Post-operative tasks Follow-up of hospitalized patients:

Transport of the patient

·       Organize and assist safe transport of the patient from the operation room to the recovery room.

·       Is responsible for the nursing care of the patient till the handover is completed.

·       Install the patient in the recovery room with the help of the recovery nurse.

·       Hand over the patient to the ward nurse and brief essential information for a good follow-up, monitoring, and care of the patient.

Material collection

·       Collect the surgical instruments that were previously wiped clean, checked, and sorted out by the scrub nurse.

·       Respect standard precautions and MSF recommendations for the collection.

·       Is responsible for:

-  Sending the used surgical and anesthetic materials to the sterilization room

-  Good waste triage and management according to MSF protocols.

·       Put the operation room back in operational state:

-  Cleans and disinfects surfaces and medical equipment are followed and accomplished, according to MSF protocols.

-  Ensure all instruments and equipment are again available and ready-to-use in the operation room.

-  Re-supply the OT with linen, instruments, sets, medical supplies, and small equipment.

-  Prepare the OT to be able to face the next surgical intervention or any emergency.

Management of written/oral handovers:

·       Maintain registers and files up to date at all times, and enter all pertinent information.

·       Fill in the handover book, note everything that happened during the shift and any changes in prescription or patients to be followed up

·       Ensure an oral handover at every team changeover, draw attention to any notable events during the shift

·       Participate in the medical team meeting to exchange information on the state of the patients.

·       Notify any problems to the direct Supervisor.

Management of material and consumables:

·       Strict management of material and consumables.

·       Daily maintenance of material and medical/surgical supplies provided

·       Contributing to proper management of stocks and renewing orders.

Reporting:

·       Report immediately to the technical referent any problem arising in the service, especially the loss/ robbery/damage of medical equipment or medicines.

Job Requirements

Education

•          Nursing degree/diploma with a specialization as a theatre nurse is essential.

•          Bachelor’s in science of Nursing is desired.

•          Current nursing license in Palestine.

Experience

2 years of clinical OT nursing is required. Having worked in MSF is highly desirable.

Language

English is required.

Competencies

·       Good organizational skills and ability to work under pressure.

·       Adequate computer skills.

·       Self-motivated and willing to learn from others.
