Background:

 

MAP is launching a mobile clinic project to provide primary health care services for Area C communities.  The target communities suffer from increased demolitions of infrastructure and lack of Israeli-required building permits, which affect the housing conditions, access to water and access to roads, all contributing to the deterioration of their health status.

 

The mobile clinic is planned to provide the following services:

Screening and treatment services

Lab tests

Medications distribution

Patient’s referral to other health service providers

Home visits

Antenatal cate and family planning

Health awareness and education

MHPSS

 

Objective:

 

The patient record system will be used to manage patients’ information at the mobile clinic location, record the provided services and track the medication distribution. 

 

Scope:

 

The mobile clinic requires:

an automated patient record system that enables the team to enter and extract data efficiently.

in addition to facilitating the clinic operations, this system will be a source for data for long term research on the impact of MAP’s intervention on the health of the targeted communities.

The patient record system will include the following:

Patient’s personal information (location, age, gender, and disability)

Diagnosis

Prescription

Medications received at the clinic

Prescribed lab tests and their results

Patient history recording

Registration of receiving additional services such as MHPSS sessions or health education sessions.

Linkages between medication inventories and distribution logs.

Data extraction and analysis.
