Job Title: Psychologist Case Manager

Scope of Work:
-Assume case manager responsibility as part of providing case management and mental health services attached to select Primary Health Care (PHC) clinics.
-Hold routine daily coordination meetings with other members of case management team and clinic staff.
-Perform Initial Assessments of referred clients and communicate results to the case management team and PHC staff.
-Plan, coordinate, and implement a Comprehensive Assessment for each new client with the case management team.
-Coordinate development of an individual treatment plan for each client with the case management support team.
-Assist in the development of list/network of CBOs and other providers of services relevant to clients presenting at the PHC level.
-Link, coordinate, and monitor implementation of the treatment plan with case management team, partner institutions, community mental health clinics, and other community organizations.
-Monitor, document and evaluate client progress throughout the treatment plan.
-Repeat Comprehensive Assessment for each clients periodically (every 3 months)
-Liaise with case management psychiatrist for clients in need of more specialized services.
-Participate in weekly supervision and coordination sessions with case management team.
-Participate in training and case consultations with PHC clinic staff.
-Submit weekly reports of activities and client outcomes to IMC Program Manager
-Provide MH services (e.g. CBT, Stress Management & problem solving)
Deliverables:
-Weekly reports of all activities and client outcomes (e.g. functioning, symptoms, well-being) and client feedback (e.g. satisfaction with services)
-List/network of CBOs providing services to individuals with mental illness
-Facilitation of client service provision
Minimum Qualifications :
-Master Degree in Psychology and/or Diploma in mental health or Bachelor degree in Psychology with sufficient experience & knowledge.
-At least 2 years of progressive experience in working with individuals suffering from mental illness.
-Excellent technical skills & knowledge
-Experience in teaching or training
-Excellent knowledge of & connection to community organizations
-Professionalism and very good communication and presentation skills.
-Excellent skills in building and maintaining relationships.
-Good English skills
-High motivation to learn
-Strong commitment to serving individuals with mental illness
